Who It's For

The Aftercare Resource Centre is for young adults aged 16 to 25 who are preparing to leave out-of-home care services, or who have recently left their care arrangements. This service is offered face-to-face, but is also available online via Zoom or by phonecall.

How We Help

We can provide you with support and information that will enable you to start the process of transitioning to independent living. Our services are free, confidential, and participation is voluntary.

What to Expect

You’ll meet with a professional caseworker who’s highly experienced in aftercare issues. They’ll discuss your current situation and provide tailored support for your individual circumstances.

Care leavers can often experience a range of unique challenges.

To refer someone to this program, please download and complete the referral form below. If you are interested in booking for yourself, no referral form is required.

How We Can Help:

Assisting you with your Leaving Care Plan and Transition to Independent Living Allowance (TILA) funds

Support with finding housing and setting up your home

Discussing Centrelink, education and employment pathways with you

Helping you access legal and financial assistance

Accessing care files and offering support while you read them

Supporting you with learning life skills, including parenting and budgeting

Referrals to other health services, including counsel appropriate agencies
08
Providing support if you wish to access the Victims Compensation Scheme
