Reconnect Wellbeing Support

The Reconnect program is for older adults, who are experiencing loss, isolation and disconnection as a result of the COVID-19 pandemic.

Who is it for?

The program is for people 65 years or older, and 55 years or older for Aboriginal and Torres Strait Islander people, who are experiencing social isolation or loneliness and at risk of distress, mental ill-health or have wellbeing and mental health issues.

This includes carers over 65, or Aboriginal and Torres Strait Islander carers over 55.

Some may be experiencing varying levels of loneliness and require assistance in reconnecting with their social networks and supports or assistance in managing low levels of situational distress.

Others may have more intense needs associated with ongoing mental ill-health, which may require counselling or referral to more specialised support.

What will it be like?

Our services offer free support without judgement or discrimination in a safe and supportive environment. The service will help to:

Connect people over 65 to appropriate resources, services, networks and support.

Restore access to services and support networks that may have been disrupted by the pandemic.

Provide information, support and counselling to improve the mental health and wellbeing of the person, their family and carers

Offer professional casework support (COVID safe face-to-face, online and over the phone).

Facilitate access to interpretation or cultural translation services.

Support referrals to appropriate specialised services.

Everyone’s experience of loss, isolation and disconnection is different as are their needs. Our caseworkers are client-focused and there to listen and support.

  • Improve wellbeing.
  • Increase connection and confidence.

How can it help?

Reconnect will provide face-to-face, telephone and/or online information and support to socially-isolated older people and their carers in their own home or at a mutually convenient and acceptable venue. We can help to support:

  • reduced social isolation/loneliness
  • improved sense of wellbeing
  • increased sense of connection/confidence.

Our staff

Our Reconnect Wellbeing Support Workers have professional qualifications and experience in social work, psychology, mediation, law and/or counselling. Staff providing the program receive professional supervision and ongoing training.

Locations

Home visits and community support is available to those living in Central & Eastern Sydney PHN areas of Sydney which includes: Inner City, Eastern Suburbs North, Eastern Suburbs South, Canada Bay, Canterbury, Leichhardt, Marrickville-Sydenham-Petersham, Strathfield-Burwood-Ashfield, Botany, Hurstville, Kogarah-Rockdale, Cronulla-Miranda-Caringbah and Sutherland-Menai-Heathcote.

If you would like to receive a service outside of these times, please get in touch and we will do our best to find a time that works for you.

You can refer yourself (if you are 65+, Aboriginal and Torres Strait Islanders 55+) or anyone else can make a referral for the Reconnect program. Download and complete the referral form and return to enquiries@ransw.org.au. We will be in contact with you to follow up on the referral.
